Thank you for filling out our intake form. At this time, our Family Health Team is Dedicated to supporting residents who do not already have a primary care provider. Since you indicated that you already have one, you are not eligible to join our team.

We encourage you to contact your current Primary Care Provider for any medical needs care because:

• They know your story: Your Primary Care Provider understands your medical history, past treatments, and health goals.
• They have your records: Staying with them helps avoid repeating tests or missing important information.
• They can coordinate your care: From referrals to specialists to follow-up care, your Primary Care Provider is your main point of connection in the healthcare system.

We know it can sometimes be hard to find the right care. If things change in the future and you no longer have a primary care provider, you are always welcome to return and see if our Family Health Team is accepting new patients at that time.
