Here’s How We Can Help

Thank you for completing our intake form. At this time, our Family Health Team can only accept patients who live within our designated service area. Because your postal code is outside that area, you are not eligible to join our team.

Our Family Health Team is part of Ontario’s healthcare system, which organizes care through Ontario Health Teams (OHTs). Each OHT focuses on supporting patients within a specific geographic region.

By caring for patients who live in our service area:

  • We stay coordinated with local hospitals, specialists, and community services that are part of our OHT.
  • We make care easier to access by connecting you to nearby services, labs, and programs.
  • We ensure fairness so patients living in our community can rely on timely access to care.

Because you live outside of our OHT’s region, we recommend connecting with a Family Health Team or primary care provider closer to your home. This helps make sure your care is well connected to the right local supports.

What You Can Do Next:
  • Contact a nearby family doctor or clinic — they may be accepting new patients.
  • Ontario’s Health Care Connect(HCC) program can help match you with a provider in your area,click and register today.
  • Community Health Centres (CHCs) — many CHCs provide care based on postal code and may serve your neighborhood. Find your closest CHCs

  • If your health concern is urgent (such as chest pain, trouble breathing, or sudden severe symptoms), please call 911 or go to your nearest Emergency Department.
  • For urgent but non-emergency medical advice, you can call Health811 (dial 811) to speak with a registered nurse, available 24/7.
Walk-In Care at Get Well Clinic:

If you are willing to travel outside your area and need care for an ongoing concern, you are welcome to use the Walk-In Clinic at Get Well Clinic. Our walk-in service can help with:

  • Minor illnesses and injuries
  • Prescription renewals
  • Ongoing issues that cannot wait until you see your primary care provider

We know this may be disappointing, but we want you to have the best possible care — and that means being connected to providers and services within your local OHT. If you move into our service area in the future, you are welcome to apply again.
