Not Eligible – Your Family Doctor is Still Active
Thank you for completing our intake form. We understand that your family doctor is retiring. At this time, you are still rostered with your current provider, so you are not eligible to join our Family Health Team yet.
Once your doctor has officially retired and your care has been transferred or derostered, we will be happy to welcome you as a patient.
What you can do next:
- Mark your calendar for your doctor’s retirement date and check when you are officially derostered.
- Consider urgent or walk-in care if you need immediate medical attention before joining our team.
- For urgent but non-emergency issues, call 811
- For emergency concerns, call 911 or go to your nearest Emergency Department
- Our Get Well Clinic Walk-In Service is available if you are able to travel and need care for ongoing issues.
- Stay in touch — once your derostering is complete, you can re-submit your intake form and we’ll happily accept you as a new patient.
