Not Eligible – Your're Already connected to a Primary Care Provider
Thank you for completing our intake form. We understand you’d like to change your family doctor. At this time, patients who are already enrolled with a primary care provider are not eligible to join our Family Health Team.
Why You’re Not Eligible to Switch:
Our Family Health Team is part of an Ontario Health Team (OHT) — a model designed to coordinate care within specific communities in Ontario.
Under this model:
- Each patient is connected to one primary care provider or family health team at a time.
- This ensures your care is coordinated and consistent, with your medical history kept in one place.
- Switching between doctors or teams isn’t generally permitted unless your current family doctor retires, relocates, or closes their practice.
Because you are already rostered with a family doctor, you remain ineligible to join our Family Health Team at this time.
What you can do next:
- Stay with your current family doctor for regular and ongoing care.
- Talk to your doctor if you have concerns — sometimes communication can help resolve issues.
- Use walk-in services if you need care outside of your doctor’s hours:
- Our Get Well Clinic Walk-In Service is available if you’re willing to travel.
- For urgent care:
- Call 811 for medical advice
- Call 911 or go to your nearest Emergency Department in an emergency
We know it can be frustrating if you’re unhappy with your current care. Please know that you’re not alone — many patients feel this way at times. For now, the best option is to continue with your current family doctor while using walk-in or urgent care services when needed.
