

North York Community Care Clinic
This clinic is for patients without a family doctor and wish to be attached to a Nurse Practitioner for your primary care needs for the time being. This program is a collaboration between North York Toronto Health Partners, Ontario Health Team (NYTHP OHT), and Get Well Clinic. And is funded by the Government of Ontario.
Eligibility Criteria
To be eligible for our clinic:
- You do not have an existing family doctor or nurse practitioner
- You live in North York within our geographic boundaries (see below)
- You are able to travel to the clinic for in-person care either at
- Get Well Clinic (649 Sheppard Ave WEST, Toronto, M3H 2S4)
OR - Baycrest Hospital (3560 Bathurst St, Toronto, M6A 2E1)

What is a Nurse Practitioner?
Nurse practitioners (NPs) are registered nurses with advanced training and clinical experience who can diagnose and treat illnesses, order and review tests, prescribe medication, make referrals, and perform medical procedures. They can advise on physical and mental health supports and work closely with other providers for a whole-person approach to care.

Conditions for Participation
In order to join our Nurse Practitioner Primary Care Clinic, you are willing to comply with the following conditions, which enable us to provide you better integrated care across the health care system:
Your primary care needs will be provided by a team of health care providers, led by a Nurse Practitioner, and may involve other healthcare team members.
You have the choice to participate in our chronic disease management programs, where you care may be provided by a team of integrated health providers.
We are a teaching site; as such medical learners may be involved in your care.
Your health care and sociodemographic information may be shared with other members and organizations of an Ontario Health Team such as North York Toronto Health Partners, for referral and care purposes.
Your medical information may be used for quality improvement research, health system monitoring, planning, management and evaluation purposes.
You will notify us when you have found a permanent family doctor or alternate nurse practitioner clinic elsewhere.
